For your first visit to our pratice,
Most of the time, your doctor or dentist will refer you to us. We need the current address of every patient and information on their medical history and health status. It is also important for us to know which medications you take regularly. This information is recorded in the admission questionnaire. Please bring the following documents, if available:

• Physician referral letter
• Health insurance card
• Photo ID
• Medication plan
• Doctor’s letter
• Current X-ray images in optimal quality (please no grainy/pixelated printouts and no email attachments with reduced data!)

You can download our forms here, which you can use to prepare for your visit to our practice. You can also have these sheets handed over to you when you visit us. We would be happy to discuss any questions with you personally in our practice.
